A spreadsheet from HMRC summarising what your Self Assessment returns say for each year of the McCloud remedy period (2015/16 to 2022/23): taxable income, tax relief claimed, annual allowance figures and scheme pays. HMRC sends it by email once you confirm you accept their email protocol. It is the HMRC document our PLUS import reads, and it is what lets a remedy calculation be done without re-keying eight tax returns.

HMRC's Public Service Pensions Remedy team send this as an Excel spreadsheet by email, so the wording includes the HMRC email protocol (CC/FS72) confirmation they need before they will reply. Put your own name in the subject line ("PSPR submission request - your name") — it is how they file the request. HMRC will not reply until they have their completed proforma and your agreement to their email protocol — so use the Download completed proforma button above (your details, email address, signature and date are filled in) and attach it to this email. You must give either your National Insurance number or your Unique Tax Reference, and list any previous addresses from the last five years (write "None" if you have not moved). Send it from the email address you want the extract delivered to. Replies can take several weeks. If you did not complete Self Assessment for some of the years, HMRC will simply return blanks for those years.
